Red level of epidemic danger! How to submit documents?

Dear visitors!
Due to the red level of epidemic danger establishment in Kyiv from 12 a.m. on November 1, 2021 because of the spread of acute respiratory disease COVID-19 caused by coronavirus SARS-CoV-2 (Minutes of an extraordinary meeting of the State Commission on Technogenic and Environmental Safety and Emergencies from 28.10.2021 № 42), from 01.11.2021 until another decision is taken, the reception of citizens is carried out under the following conditions:
- visitors shall wear personal protective equipment, in particular, respirators or protective masks covering the nose and mouth, including self-made ones;
- visitors shall observe at least 1,5 meters distance;
- visitors shall hold one of the following documents (except for those under 18 years of age):
- negative result of testing for COVID-19 by polymerase chain reaction (PCR) or rapid test for coronavirus antigen SARS-CoV-2, conducted no later than 72 hours before the visit to our office,
- document confirming the completion of the full course of vaccination, international, domestic certificate or foreign certificate confirming vaccination against COVID-19 with one dose of single-dose vaccine or two doses of two-dose vaccine (green certificates), included in the WHO Emergency Use Listing,
- document confirming the negative result of testing by polymerase chain reaction or recovery of a person from the disease, the validity of which is confirmed by the Unified State Web Portal of Electronic Services, in particular using the mobile application of the Portal Diia.
